What is the Coming Home Program?
Coming Home is here to meet you where you are, offering deeply affordable housing, personalized support, and meaningful community connections.
We provide individuals and families with stable housing in an apartment community, along with access to supportive resources that promote long-term stability, personal growth, and a sense of belonging.
How We Help
As a resident in our Coming Home program, you’ll have access to:
- Personalized support from a Resident Services Coordinator who understands the unique needs of individuals and families exiting homelessness or transitioning from short-term or unsustainable housing.
- Coaching and guidance to help you reach your goals—whether you’re seeking employment, pursuing education, rebuilding your health, or looking for a safe and supportive place to age in place.
- Community Connections through events, activities, and opportunities to build relationships with others and feel a true sense of belonging.
Who Can Join the Coming Home Program?
Applicants must meet initial eligibility requirements and be considered “Program Ready” to join the housing waitlist for the Opportunity Home sites.
- Hawthorn–(specific 2- and 3-bedroom units)
- Manhattan (specific units)–applicants must be 62 years or older or under 62 years with a disability
- Northport (specific units)–applicants must be 55 years or older
- Walnut Place (specific units)–applicants must be 62 years or older
- Hilltop (55+–specific units)–applicants must be 55 years or older
- Rally (specific units)–single person must be either 62 years or older, a person with a disability, or a family (specific units)
Each of these communities has access to resources, including community spaces with libraries, computers, Wi-Fi, and onsite programming and community events.
Supportive services start when you are accepted into the Coming Home program and admitted into a housing community.
Eligible residents pay approximately 30-40% of their income towards rent.
Who Should Apply?
Low-income people with a disability who live or work in Boulder County. There is no minimum income required to qualify, and applicants may earn up to 50% of the Area Median Income to qualify:
